I finally found a fix that has solved the problem (it’s been working for four days as I type this comment).

I went into Airport Extreme wireless options and changed 2.4GHz from Automatic to Channel 11, so the Airport doesn’t rotate the channel. After doing that, my iPhone Remote app works perfectly.
